For an engineering report on any wells listed on EnergyNet.com, please fill out and print this form for every well you wish evaluated. Information above 'Lot #' needs only to be filled in for the first well. Please fax your completed forms to GSM at (806) 358-6800. The GSM Reservoir Engineering hourly rate for registered EnergyNet Buyers and Sellers is $175/Hr. However, a ten percent discount will be given to EnergyNet customers for properties listed on EnergyNet.com. A minimum of one hour of evaluation time will be charged for each evaluation package regardless of the number of wells evaluated. GSM estimates that a single well, which is already listed on the EnergyNet.com website - either under review or up for auction - will require about 1/2 hour of work to render a reserve and cash flow evaluation.
